Placing fillers in patient’s faces for 30 years, BRUISING after treatments is one of the most annoying and upsetting problems to our patients as well as ourselves.

Creating a blunt-tipped flexible cannula similar to the instruments that we use when performing structural fat grafting to the face, only smaller, is a revolutionary breakthrough in filler treatments. The technology has improved to the point that a very fine, long blunt “needle” or more accurately, a cannula, can be manufactured at a reasonable price that is disposable. These new devices can be used for Juvederm, Restylane, Radiesse, Sculptra, and virtually any other kind of filler treatment, including fat grafting! Although you still can get some bruising, we are sure the reduction will be more than 50%! Also because the “needles” are blunt, the discomfort during the procedure is much less! This is a win-win situation for everyone. We have been very happy using these new fine blunt cannulas and feel that We can produce better results with less bruising and less discomfort.

Latisse®

LATISSE® (bimatoprost ophthalmic solution) 0.03% is the first and only prescription treatment approved by the FDA for inadequate or not having enough eyelashes, growing them longer, fuller and darker.

LATISSE® makes lash growth possible because of its active ingredient: bimatoprost. Although the precise mechanism of action is not known, research suggests that the growth of eyelashes occurs by increasing the percent of hairs in, and the duration of, the anagen (or growth) phase. Lashes can grow longer, thicker and darker because bimatoprost can also prolong this growth phase.

LATISSE® is FDA-approved and effective. It’s a once-a-day treatment you apply topically to the base of your upper eyelashes. Patients in a clinical trial saw results in as few as 8 weeks with full results after 12 to 16 weeks.

While LATISSE® is a treatment for inadequate or not enough eyelashes, it was developed through years of research by Allergan, a pharmaceutical leader with over 60 years of expertise in prescription eye care products.

Restylane®

Restylane® is approved for the treatment of fine lines around the mouth and lips, smile lines, marionette lines, and frown lines. Restylane® can also be used to fill out depressed areas beneath the lower eyelids. Restylane® is one of several new injectables made of hyaluronic acid, a natural substance that is produced by the body and naturally absorbed over time. Cosmetic surgery patients in Mountain View can expect Restylane® injections to last for approximately 4-8 months, after which time-periodic sessions will be required to maintain the effects of the treatment. Restylane® treatments can cause significantly more discomfort than BOTOX® injections. Dr. Ereso has found that local anesthesia placed in the treatment areas prior to placing Restylane® has dramatically improved patient comfort during the procedure. We recommend that patients ice the treated area after the injections, and allow 1-3 days for bruising, redness, and discomfort to subside.

SCULPTRA AESTHETIC® – a Breakthrough Volumizer

A new breakthrough anti-aging product has been developed that actually adds volume back to the aging face. It is called SCULPTRA Aesthetic®. Developed in Europe in 1999, Sculptra was introduced to the United States in 2004. In 2009, SCULPTRA Aesthetic® was FDA approved as an anti-aging cosmetic enhancement formula.

dermal filler san joseWhat is SCULPTRA Aesthetic® and how does it work? As we age, we lose facial volume combined with increasing laxity of the facial skin resulting in the appearance of an aging face–sagging jowls and neck skin in combination with hollowness around the eyes, cheeks, and brows. In addition, smile lines deepen and marionette lines around the mouth become more apparent. SCULPTRA Aesthetic® or poly L lactic acid (a complex sugar), is placed deeply in the cheeks, temporal areas, smile lines and marionette lines. It can also be used with caution to improve the hollowness around the eyes and the tear trough area beneath the eyes. After placing the SCULPTRA Aesthetic®, the body slowly replaces the poly L lactic acid with newly formed collagen. Gradually, your own body’s collagen restores much of the lost volume to the areas treated resulting in a refreshed, more youthful appearance without any scars or surgery. Usually, 3 treatments, performed 6 weeks apart, are necessary to achieve the desired results. The treatment program is very flexible and is easily customized for your individual needs and requirements. You can expect your results to last at least 25 months and frequently improvement in your appearance can be noted for 3 years or longer! Dr. Ereso personally performs all of the SCULPTRA Aesthetic® procedures.

A great deal of attention has been placed on the safety of using SCULPTRA Aesthetic®. Initially, reports from Europe of nodule formation in the cheeks and other areas kept our doctor from using this product on his patients. Nodule formation has been minimized by diluting the SCULPTRA Aesthetic® with sterile water, which reduces clumping of the poly L lactic acid as well as by placing the product in a deeper plane, farther away from the skin surface. In addition, 2cc of lidocaine, a local anesthetic, is added to each vial of SCULPTRA Aesthetic®, which reduces the discomfort level of the procedure to a minimal level. You should expect 1-3 days of swelling and some bruising after each treatment session.

Overall, SCULPTRA Aesthetic® is safe and effective in restoring volume to the aging face producing a more youthful appearance. With a result that lasts at least 2 years, it is the longest-lasting product that is safe. Dr. Ereso, one of the leading plastic surgeons in the Bay area, is an excellent choice to perform this procedure.

Radiesse®

Radiesse® is a fantastic new FDA-approved filler that is primarily composed of hydroxyapatite suspended in a water-based gel. It feels thicker and denser than hyaluronic acid fillers (Restylane® and Juvederm™) and is placed in a slightly deeper plane of tissue. It is an excellent way to fill deep smile lines, furrows and marionette lines that extend below the corners of the mouth. It can also be used effectively in the cheek area and jawline to restore volume. We do not recommend Radiesse® for the lips at this time. Usually, a local anesthetic is placed in the area prior to treatment. As with Restylane® and Juvederm™ treatments, some swelling, bruising and discomfort are to be expected.

An initial complete treatment frequently consists of a touch-up dose administered three to six months after the first Radiesse® is given. It is not uncommon to see significant improvement for 10 months or longer after the initial treatment is finished. Our patients and our team have been extremely impressed with the outstanding results from this exciting new filler.

Other Dermal Fillers

ArteFill® is a newer version of Artecoll® (methylmethacralate microspheres in a gel base). ArteFill® and Artecoll® appear to have the longest-lasting results (1-2 years) but have occasionally resulted in nodules (granulomas) that do not appear, sometimes, until many months after the procedure is performed. They can be somewhat difficult to treat. We have decided not to use these products until longer-term results and improved safety have been proven to us.

The “Fraxel®” Laser

We are excited to be one of the first practices in the San Jose Bay Area to offer the Fraxel® Laser to our patients – (www.reliant-tech.com). This revolutionary innovation allows the areas treated to exhibit better skin texture, more even pigmentation, less wrinkling and fine lines, as well as some tissue tightening. We feel it is the treatment of choice for age spots, especially on the face and hands, as well as melasma and freckling. There is minimal “down” time (1-2 days of mild redness), and the procedure does not require general anesthesia or intravenous sedation, making it an ideal option for San Jose Bay Area residents.

What to Expect

We ask that you arrive at the plastic surgery center one hour prior to your scheduled treatment. A topical anesthetic will be liberally applied to the planned treatment areas. This will be removed approximately 45 minutes later. A blue vegetable dye will then be applied to the skin, followed by additional local anesthetic ointment. The blue color helps the Fraxel® Laser “recognize” the proposed areas for treatment. The laser will then be used to thoroughly treat all of the planned areas. You will feel some heat during the procedure, which will be minimized by a chilled spray applied to the treated sites. Redness and some stinging follow. Iced compresses and elevation significantly reduce discomfort. Redness usually disappears in several hours but may last for 1-3 days. Normal activities may usually be resumed in less than 24 hours.

How the Technology Works

The Fraxel® Laser treatment concept is ingenious. For every square inch of treated area, only approximately 30-50% or “fractions” of the tissue is actually exposed to the laser light producing heat and tissue changes to the deeper structures without injuring the skin surface. Most of the area surrounding the lasered tissue is left undisturbed allowing for rapid healing and minimum downtime. You should expect to see improvement in the texture, color, and fine lines in the treated areas over a period of 1-3 months. Usually, 4-5 treatments are required to achieve the desired results and clinical improvement may not be apparent until 2-3 treatments have been completed. The treatments may be spaced 1 to 4 weeks apart.

What Areas Can Be Treated

One of the most exciting aspects of this new laser treatment is that almost any area of the body can be treated. Typically, an entire face, neck, and chest can be treated in one session. The hands and entire upper extremity can be treated, as well as the legs, ankles, and feet! Another fabulous advantage of the Fraxel® Laser treatment is that skin of “all colors” may be treated. Asian, Mediterranean, Hispanic, African-American, and Caucasian!

Safety

Because SCULPTRA Aesthetic®, Restylane®, and other hyaluronic acid-based dermal fillers are not derived from animal or human origin, they minimize the risk of allergic reaction or transfer of disease. These fillers are composed of natural substances that are gradually absorbed by the body after 4-8 months. Side effects are minimal, and no pre-testing is required before treatment.
